INSTRUCTIONS |

1 | You must fill in every blank with the exception of the RMA number which Thermco will assign once we receive the request. Carefully fill in your correct email address or we will not receive the request. |

2 | Once you have the form filled completely in, be sure to PRINT it before you hit the "Submit" button and keep this copy for your records. Don't forget to PRINT the shipping label. |  |

3 | Once Thermco receives the request, a technician will evaluate it and then phone or email the contact person. |  |

4 | You will be given the RMA number which you can then write onto the page you previously printed out. You will also be asked for a Purchase Order number at that time. |  |

5 | You can then proceed by making a copy of your RMA. This you will enclose in the package with the item you are returning. Use the shipping label you printed out and afix it to the outside of the package. |  |

6 | All gas analyzers must be placed in a plastic bag and wrapped with at least 4 inches of bubble wrap on all sides or damage may occur. |  |

7 | Mini-gas mixers should be shipped with at least 2 inches of bubble wrap on all sides and placed in a strong carton or placed on a skid. |  |

8 | Large gas mixers should be bolted to a strong skid with 4 strong bolts through the surge tank feet and wrapped in heavy duty plastic. |  |
